The Golf Cooperative – Guest Check-In & Liability Waiver All guests must complete this form before using The Golf Cooperative facilities. Guests are the responsibility of the hosting member and must comply with all cooperative rules.
Guest Rules & Responsibilities
1. Guests are the responsibility of their hosting member at all times.
2. Guests may not be left unattended in the facility under any circumstances.
3. The guest day pass fee of $40 covers up to 4 hours of facility use and must be paid prior to play.
4. All guests must sign this liability waiver before participating.
5. Guests may only enter and use the facility when accompanied by their hosting member.
6. Guests must adhere to all rules, policies, and the Code of Conduct of The Golf Cooperative.
7. Guests must use equipment safely and as instructed.
8. Guests and/or their hosting member are liable for any damages caused by the guest.
9. No alcohol, drugs, or tobacco use is permitted on the premises.
10. Guests under 18 must be accompanied by an adult or authorized coach at all times.
11. Each guest is limited to six (6) guest visits per calendar year before membership is required.
12. Guests must not film or photograph others without their consent.
13. The Cooperative reserves the right to refuse or revoke access for violations or unsafe behavior.
